The best approach to develop functional capacity is through exercise. Early normalization activation, which includes exercise, should start for patients with acute back pain in the primary care environment. Unfortunately, many primary care doctors do not advise their patients with back discomfort to exercise. Exercise-directed therapy is well-supported by research for people with persistent back pain. Skeletal muscles go through a process known as adaptive reductive remodeling in response to inactivity. This causes a decline in muscle strength due to a loss of muscle mass and changes in the composition of the muscles (see sarcopenia). Erector spinae, transversospinalis, and abdominal muscles atrophy and increase in fat content in response to physical inactivity, increasing the risk of low back discomfort.
